The Seoul-based Open Radio for North Korea says the condition worsened last month and blames it on what it calls Kim Jong Il's "excessive smoking and drinking."
Still, the report adds that the 67-year-old leader has recovered greatly from last year's reported kidney disease and stroke.
The station cites an unidentified "high level source" in the North.
Kim's health is a focus of intense media attention because he rules absolutely and there could be a power struggle in the North if he were to die without naming a successor.
South Korean officials could not confirm the report.
